New Client Information

Please click the appropriate button below to download and print the new client forms. Or you can come in and complete these forms before your first appointment. We recommend arriving at least 30 minutes early to complete forms.

Paperwork MUST be completed before your first appointment.

Please read, complete, sign and return to the receptionist or your therapist before or on the date of your first session. If you have any questions regarding filling out forms, please ask and we will be happy to answer your questions.

If you have an insurance card, have the receptionist or your therapist make a copy of your card so we have it on file.

Rates, Billing, and Insurance 

Rates
We charge $150 for the initial assessment and $130 for every session afterwards. For your convenience, we do bill insurance. 

Insurance
We accept most insurances. Most insurance companies offer “In” and “Out” of Network benefits, but not all insurance companies offer “out of network” benefits. You may call your insurance to check if Pathways is covered by your insurance. You may find it helpful to ask about your coverage with the following questions:

• Do I have mental health insurance benefits?

• What is my deductible and has it been met?

• How many sessions per year does my health insurance cover?

• What is the coverage amount per therapy session?

Unfortunately, we are not Medicaid/Medicare providers. We bill Primary Insurance Only.

Self-payment/Non-Insurance
Services are available for individuals who do not have or do not want to utilize their insurance coverage. Please contact us directly to discuss session rates and payment plans.

Reduced Fee
Reduced fee services are available on a limited basis.

Payment
Cash, check, and credit cards accepted for payment.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you may be required to pay a cancellation fee.

Confidentiality & Privacy Policy

The law protects the relationship between a client and a psychotherapist, and information cannot be disclosed without written permission.

Exceptions include:

• Suspected child abuse or dependant adult or elder abuse, which we are required by law to report to the appropriate authorities immediately.

• If a client is threatening serious bodily harm to another person/s, we must notify the police and inform the intended victim.

• If a client intends to harm himself or herself, we'll make every effort to enlist their cooperation in insuring their safety. If they do not cooperate, we will take further measures without their permission that are provided to us by law in order to ensure their safety.
