Please read, complete, sign and return to the receptionist or your therapist before your first session. If you have any questions regarding filling forms, please ask and we will be happy to answer your questions.

If you have an insurance card, have the receptionist or your therapist make a copy of your card so we can call and verify coverage.

Rates, Billing & Insurance Questions

Insurance
We accept most insurances. Most insurance companies offer “In” and “Out” of Network benefits, but not all insurance companies offer “out of network” benefits. Unfortunately, we are not Medicaid/Medicare providers. We bill Primary Insurance Only.

Self-payment/Non-Insurance
Services are available for individuals who do not have or do not want to utilize their insurance coverage. Please contact us directly to discuss session rates and payment plans.

Reduced Fee
Reduced fee services are available on a limited basis.

Payment
Cash, check and credit cards accepted for payment.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you may be required to pay a cancellation fee.
